It was a bug in a firmware release that caused those, it could happen if you tried copying one, it did happen if you added a description to a template.
Upgrade to the latest release here and it hopefully it will allow you to delete the entry after. Otherwise an Altboot would be required to fix it, theoretically a clone file could be used to restore the device, but you would want to make sure Workflow scan settings were not in the clone or you would be right back where you are now after they were restored.
