First, set up a template to get the settings right instead of having to test repeatedly with faxes, it will make it much easier.
1. Don't use the hostname, that won't work, this device is a "Connectkey" device, they don't support WINS or Microsoft Networking or NetBIOS over TCP/IP, which means if the device is not registered in DNS on a Domain, hostname resolutions will not work.
2. Due to the above, if scanning to Server 2008/2012 you will get a login error if you do not format the username exactly as it is shown on the share in this window (It has to be exactly as shown in the brackets, so add the user directly if it is not already) It doesn't matter how other devices are connected, due to number 1 up above, this one requires this setup.
If, when you create a template with SMB, the port lists as 139, it is very likely you will require a firmware update, it should be 445 for any typical OS running these days (SMB2-3)
Assuming you can reach the location you list from a PC on the network with the share by clicking on the Start menu and typing \\OurServer\FaxShare then the following screenshot is what yours should look like to be successful with exception to the IP address and username obviously being different.
The latest firmware for your device is here, if all the above fails to work, you would need the update.
